The Problem with Missing Teeth

Losing a tooth is not merely a cosmetic issue. Missing teeth cause a cascade of changes: adjacent teeth tilt into the gap, opposing teeth over-erupt, chewing efficiency decreases, speech may be affected, and — importantly — the jawbone beneath the missing tooth begins to resorb (shrink) within months of tooth loss. Long-term bone loss changes the shape of the face and can make future tooth replacement more complex.

Conventional Dentures

Dentures are removable prostheses — acrylic (plastic) teeth set in an acrylic or metal base that rests on the gums. They come in two forms: complete dentures (replacing all teeth in an arch) and partial dentures (replacing some missing teeth).

Advantages of dentures:

  • Lower initial cost than implants
  • No surgery required
  • Can be fabricated relatively quickly
  • Suitable for patients who are not candidates for surgery

Disadvantages of dentures:

  • Rest on the gums — can slip, especially during eating or speaking
  • Do not prevent bone loss — the jawbone continues to resorb, causing the denture to become loose over time
  • Reduced chewing efficiency compared to natural teeth or implants
  • Must be removed at night and cleaned daily
  • Require periodic refitting or replacement as bone shape changes
  • Reduced taste sensation (particularly upper full dentures which cover the palate)

Dental Implants

A dental implant is a titanium screw placed into the jawbone, mimicking the root of a natural tooth. After the implant integrates with the bone (osseointegration — typically over 3–4 months), a ceramic crown is placed on top, creating a restoration that looks, feels, and functions like a natural tooth.

Advantages of dental implants:

  • Fixed — does not move during eating or speaking
  • Preserves bone — the implant stimulates the jawbone like a natural tooth root, preventing resorption
  • Chewing force close to natural teeth — 80–90% of natural bite force restored
  • No removal — cleaned like natural teeth with brushing and flossing
  • Long-lasting — implant fixture can last a lifetime with proper care
  • Does not affect adjacent teeth — unlike a dental bridge, which requires grinding down neighbouring teeth

Disadvantages of implants:

  • Higher upfront cost than dentures
  • Requires surgery under local anaesthesia
  • Longer treatment timeline — typically 4–6 months from implant placement to final crown
  • Requires adequate bone volume — bone grafting may be needed in some cases

Which Is Right for You?

For most patients who are medically fit and have adequate bone, dental implants provide a significantly better long-term outcome than conventional dentures — in terms of function, comfort, bone preservation, and quality of life. The higher upfront cost of implants is offset by their longevity and the avoidance of repeated denture refitting and replacement over time.

Dentures remain a valid choice for patients who are not surgical candidates, those with severely compromised bone who decline grafting, or those for whom the cost of implants is prohibitive in the short term. Implant-supported overdentures — where 2–4 implants are placed to anchor and stabilise a conventional denture — represent a middle ground that dramatically improves denture stability while being more cost-effective than full implant rehabilitation.
